Are you a Qualified Social Worker with a passion for making a difference? We are looking for a dedicated and compassionate Adult Social Worker to join our Transition Team in Carlisle!
Roles and Responsibilities:
- Develop and implement individualized care plans: Working closely with individuals, families, and other professionals to create comprehensive and tailored support plans that meet individual needs and promote wellbeing.
- Facilitate seamless transitions: Provide guidance, advocacy, and support to individuals transitioning between hospital, care homes, supported living arrangements, and their own homes.
- Collaborate with multidisciplinary teams: Work effectively with healthcare professionals, care providers, families, and other social workers to ensure a coordinated and holistic approach to care.
- Assess and manage risk: Conduct thorough assessments to identify and address potential risks associated with transitions, ensuring individual safety and well-being.
- Advocate for service users: Promote individual rights, ensuring individuals have access to appropriate services and resources, and their voice is heard.
- Contribute to service development: Engage in ongoing professional development and actively participate in initiatives to enhance the quality and effectiveness of the Transitions Team.
Key Requirements:
- Qualified Social Worker: Must hold a relevant Social Work qualification (e.g. DipSW, BA, BSc, MA) and be registered with Social Care England.
- Experience in adult social care: Proven experience working with adults with complex needs in a social care setting is essential.
- Excellent communication and interpersonal skills: Ability to build rapport and trust with individuals, families, and colleagues.
- Strong organizational and time management skills: Ability to effectively manage multiple cases and prioritize tasks.
- Commitment to person-centered care: A genuine passion for supporting individuals and promoting their independence.
