Adults Social Worker - Hospital Discharge - Winchester Hospital

An exciting opportunity has arisen for a Qualified Social Worker's to join our friendly and supportive hospital discharge team at Winchester Hospital.

\n

Responsibilities:

\n

\n
• Working with the acute team.
\n
• The focus will be on supporting the turn around of patients at the front door and working with the Single Point of access here at the hospital.
\n
• This role requires you to be on site 2 days a week as you will be assessing patients for discharge planning.
\n
