Key Responsibilities:
- Conduct assessments and create care plans for adults in need of social care.
- Provide ongoing support and guidance to service users, ensuring they receive person-centred care.
- Collaborate with colleagues and external partners to ensure the best outcomes for service users.
Requirements:
- Degree in social care
- Social Work Wales registration
- 2+ years experience in CWD Social Care
