Are you an experienced Social Worker with a passion for supporting adults in need?

Our client's Adults team is seeking a qualified Adult Locality AS-SG professional to join their dedicated team in the North West. This is a fantastic opportunity for a compassionate and skilled individual to make a real difference in the community while working in a flexible, supportive environment.

\nKey Responsibilities:\n
    \n
  • \nConducting Person-Centered Assessments: You'll be responsible for conducting detailed assessments and developing care plans that are tailored to the individual needs of adults in a variety of settings.
  • \n
  • \nCollaborating with Multidisciplinary Teams: Work closely with healthcare professionals and other team members to ensure integrated care and support for individuals.
  • \n
  • \nProviding Guidance and Support: Assist adults and their families in accessing the right services and resources to improve their quality of life.
  • \n
  • \nMonitoring and Adapting Care Plans: Regularly review and adjust care plans to ensure they meet the changing needs of individuals.
  • \n
  • \nBuilding Community Relationships: Engage with local communities to reduce isolation and promote social inclusion.
  • \n
  • \nEnsuring Compliance: Adhere to social care policies, legislation, and best practices while maintaining detailed, accurate records in line with data protection regulations.

\nWhat We're Looking For:\n

  • \nQualifications: Degree in Social Work (or equivalent), registered with Social Work England (or relevant professional body).
  • \n
  • \nExperience: At least 2 years PQE, with proven experience in adult social care and locality-based roles.
  • \n
  • \nSkills & Competencies: Strong assessment skills, excellent communication, time management abilities, and a genuine desire to support vulnerable adults. Familiarity with community resources, case management software, and MS Office is essential.
