About the Role

We are seeking a dedicated and experienced Adult Social Worker to join our thriving Integrated Discharge Team within the City of Doncaster Council. This is a fantastic opportunity to make a real difference to the lives of vulnerable adults in our community. You will play a vital role in supporting individuals to achieve safe and timely discharges from hospital and other care settings, facilitating their transition back home or into appropriate community-based support.

Responsibilities

  • Conduct comprehensive assessments of individual's needs and develop person-centered care plans.
  • Liaise with healthcare professionals, family members, and other stakeholders to ensure coordinated care.
  • Facilitate timely and safe discharges from hospital and other care settings.
  • Navigate complex social care legislation and procedures.
  • Contribute to the ongoing development and improvement of the Integrated Discharge Team.
  • Maintain accurate and up-to-date records.

Essential Skills and Experience:

  • A relevant Social Work qualification (e.g., DipSW, CQSW, BA/BSc Social Work).
  • Social Work England registration.
  • Proven experience working with adults with mental health needs.
  • Excellent assessment, planning, and care management skills.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Commitment to person-centered care.

Desirable Skills and Experience:

  • Experience working within a Local Authority.
  • Knowledge of relevant legislation and best practice guidelines.
