Learning Disability Team Manager
Location: Gateshead UK
Working Arrangement: Hybrid (2 to 3 days on-site per week)
We are looking for a motivated and skilled Team Manager to lead our Learning Disabilities Team. The successful candidate will oversee the team's efforts to work with adults with learning disabilities, focusing on post-preparation for adulthood (managed by our Transitions teams). This role involves managing complex assessments, care plans, and interventions across the entire Borough. The position requires a hybrid working model, with the manager being on-site at least 2 to 3 days a week.
Key Responsibilities:
Team Leadership:
Provide strong leadership and management to the Learning Disabilities Team.
Facilitate the smooth transition of the current post holder to a new role.
Ensure the team delivers high-quality services in line with best practices and organisational policies.
Case Management:
Oversee complex assessments and the development of individualised care plans.
Monitor and review care plans and interventions to ensure they meet the needs of clients.
Support the team in managing challenging and complex cases, providing guidance and supervision.
