Responsibilities:

• Provide support and guidance to adults and older people living in the community, helping them achieve their goals and maintain their independence.
• Manage a diverse caseload with autonomy, allowing for a direct impact on the lives of those served.
• Conduct thorough assessments and reviews to determine the needs of individuals, creating effective care plans that meet their unique requirements.
• Develop comprehensive care plans, ensuring individuals receive the appropriate support and resources to thrive.
• Prepare clear and detailed reports to document progress and share vital information with relevant parties.
• Work directly with individuals requiring support under the Adults with Incapacities (AWI) framework.
• Advocate for individuals who may be at risk of harm or abuse under the Adult Protection (AP) guidelines.

Requirements:

• Member of the PVG scheme
• Degree or equivalent in Social Work
• Registered with the Scottish Social Services Council (SSSC)
• Council Officer trained
• Strong knowledge and experience of the relevant legislation
