Adults Social worker- Hospital discharge - From �32 p/hr


Nonstop is currently working with a well established local authority who have a hospital based Social Work role in the Slough area as part of their Hospital Care Management team.


You will be based out of the local hospital, allowing which will allow for a lot less travelling and a more structured day with less Out of Hours being completed.

Responsibilities:


The team is made up of experienced social workers and social care assessors who work alongside doctors, nurses, occupational therapists, physiotherapists, mental health services


Work with a multidisciplinary team to facilitate appropriate, safe and timely hospital discharges for those referred to our service

Requirements


Please apply if you are a Qualified Social Worker with a minimum of 2 years experience in an adults disability/ mental health setting . You will be a Social Work degree and have a driving license.
