Key Responsibilities:

• Provide high-quality, patient-focused care in community settings.

• Conduct clinical assessments and deliver nursing interventions such as wound care, medication administration, and chronic disease management.

• Support patients and families with health education, promoting independence and well-being.

• Collaborate with multidisciplinary teams to ensure a seamless patient care experience.

• Maintain accurate, up-to-date patient records and documentation.
\n

Essential Criteria:

• Registered Nurse with current NMC registration
\n
• Previous experience in community nursing is preferred, but not essential.
\n
• Strong communication, assessment, and organizational skills.
\n
• Ability to work independently and manage your own caseload.
\n
• Flexible and reliable, with the ability to work various shifts as required.
\n
