Band 6, 7 & 8A Locum Pharmacists - Hospital Ward






  • �30-�50 per hour

  • Full-time hours

  • 3-month Locum contracts (potential to extended thereafter)

We are seeking�dedicated and experienced Band 6, 7 and 8A Locum Pharmacists to work on the hospital wards in Milton Keynes. The successful candidates will be responsible for delivering high-quality pharmaceutical care to patients, ensuring the safe and effective use of medications, and working closely with a multidisciplinary team to optimise patient outcomes. This role involves providing expert advice on medication therapy and contributing to the overall management of patient care within the hospital.





Key Responsibilities





Clinical Pharmacy Services





Conduct comprehensive medication reviews and assessments for hospital ward patients.



Develop and implement individualized pharmaceutical care plans.



Provide recommendations on drug therapy, including dosing, administration, and monitoring.



Ensure the appropriate and safe use of medications, considering potential drug interactions and contraindications.





Multidisciplinary Collaboration





Work closely with doctors, nurses, and other healthcare professionals to develop and implement treatment plans.



Participate in daily ward rounds, case discussions, and multidisciplinary team meetings.



Provide expert advice on pharmacotherapy, contributing to clinical decision-making processes.





Patient Safety and Quality Assurance





Monitor and manage adverse drug reactions and medication errors.



Ensure compliance with hospital protocols, guidelines, and regulatory requirements.



Contribute to the development and implementation of policies and procedures related to medication management on the ward.





Education and Training





Educate patients, families, and healthcare staff on medication use, side effects, and safety.



Provide training and support to junior pharmacists, pharmacy technicians, and students.



Stay current with advancements in pharmacotherapy and disseminate knowledge to the team.





Documentation and Reporting





Maintain accurate and up-to-date patient records, including medication histories, care plans, and intervention outcomes.



Prepare detailed reports on medication usage, therapy outcomes, and quality improvement initiatives.



Ensure documentation complies with legal, ethical, and organizational standards.





Research and Development





Participate in clinical research and audits to advance the field of hospital pharmacy.



Contribute to the development of evidence-based practices and clinical guidelines.



Engage in continuous professional development and reflective practice to enhance skills and knowledge.





Qualifications and Experience





Essential






  • Degree in Pharmacy from an accredited institution.

  • Registration with the General Pharmaceutical Council (GPhC).

  • At least 2 years of post-qualification experience, with significant experience in a hospital or clinical setting.

  • Strong clinical reasoning, problem-solving, and decision-making skills.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and as part of a multidisciplinary team.





Desirable






  • Previous experience in a hospital ward setting.

  • Additional qualifications or certifications in clinical pharmacy.

  • Experience in clinical research or quality improvement initiatives.
