Role Overview:
As a Band 6 Mental Health Nurse, you will work collaboratively with multidisciplinary teams to enhance patient transitions, streamline discharge processes, and ensure efficient bed management. This role is ideal for a proactive nurse who thrives in a fast-paced environment and is passionate about mental health care.
Key Responsibilities:
- Support mental health patients who are ready for discharge, ensuring a smooth transition to community or alternative care settings.
- Collaborate with ward staff, community teams, and other stakeholders to resolve discharge delays.
- Monitor and manage patient flow across mental health units, optimizing bed availability.
- Provide expert clinical advice and leadership in discharge planning and patient transition.
- Identify and address barriers to discharge, advocating for patient-centered solutions.
- Maintain comprehensive and accurate documentation in line with professional and legal standards.
Essential Requirements:
- Registered Mental Health Nurse (RMN) with active NMC registration.
- Minimum of 2 years post-registration experience, with a focus on mental health care.
- Strong understanding of discharge planning and patient flow processes within mental health services.
- Exceptional organizational, communication, and problem-solving skills.
- A team player with a compassionate and proactive approach to patient care.
