Care Assessor - Adults Learning Disabilities Social Work team

Requirements:

• Experience working with people with Learning Disabilities, Mental Health or Autism
• Knowledge of Care Act & Mental Health Capacity Acts
• Enhanced DBS
• Experience within Social Services

NonStop's client is a local authority based near Wembley is seeking a Social Care Assessor to join their Social Work Adults Learning Disabilities Team. You will deliver high-quality care to the wider community. This will mean producing high-quality work in terms of the Care Act and associated social care legislation with the ability to respond promptly to crises as well as hospital discharges.

Your duties will include:

• Carry out social care and joint health assessments.
• Devise appropriate comprehensive support plans in order to meet identified needs
• Mmonitor and review packages of care
• Undertake actions in relation to Safeguarding Adults, as required within the locally agreed Adult Safeguarding policies
