Care Coordinator

Care coordinator required for a Homecare Team�based in HX2. Full-time, permanent position.�

Applications will only be accepted from applicants with a UK driving licence with own car.

We are looking for someone who has worked in a care setting for at least 5 years, who has hands-on experience and also a minimum of 2 years experience as a leader/supervisor.

We are looking for someone who will ensure that a timely and compassionate, top quality�service is provided to all our clients. You will be the primary�liaison with our clients, relatives and carers and therefore must have an excellent command of the English language.�

You must also have an Enhanced DBS on the update service and be IT proficient.
