Job Summary:



We are seeking a compassionate and detail-oriented Case Worker to join our team. The successful candidate will play a crucial role in assessing client needs, developing care plans, and ensuring the effective delivery of home care services. This role requires excellent communication, problem-solving, and organizational skills to support both clients and care staff.



Key Responsibilities:




  • Client Assessments: Conduct initial and ongoing assessments of clients to determine their care needs, preferences, and risks.

  • Care Planning: Develop, implement, and review individualized care plans in collaboration with clients, families, and care teams.

  • Liaison & Advocacy: Act as a key point of contact for clients, families, care workers, and external professionals (e.g., social workers, healthcare teams).

  • Compliance & Safeguarding: Ensure that all care services align with CQC regulations, company policies, and safeguarding protocols.

  • Monitoring & Quality Assurance: Regularly review care plans, conduct visits, and gather feedback to ensure the highest standards of care.

  • Staff Support & Training: Provide guidance and support to care workers, ensuring they understand and follow client care plans effectively.

  • Crisis & Risk Management: Address any urgent client concerns, respond to safeguarding issues, and report incidents in a timely manner.

  • Administrative Duties: Maintain accurate records, update client files, and ensure documentation is completed in compliance with regulatory requirements.



Requirements:




  • Previous experience in a case worker, care coordinator, or similar role within the health and social care sector.

  • Strong understanding of domiciliary care, person-centered planning, and safeguarding principles.

  • Excellent communication and interpersonal skills.

  • Strong problem-solving and decision-making abilities.

  • Knowledge of CQC standards and compliance requirements.

  • Ability to work independently and collaboratively within a team.

  • Full UK driving license and access to a vehicle (preferred).

  • NVQ Level 3 or higher in Health & Social Care (desirable).
