Band 6 CPN - Community Mental Health Practitioner -

Randstad is actively seeking a highly qualified and experienced Band 6 Registered Mental Health Nurse (CPN) to join a Community Mental Health team based in Yorkshire. About the Role

The successful candidate will provide comprehensive mental health assessments, develop individualized care plans, and deliver evidence-based interventions to individuals with complex needs.

Collaboration with a multidisciplinary team, including social workers, GPs, and therapists, is paramount.

This position demands proficiency in conducting home visits and providing community-based support, ensuring person-centered care.

Maintaining meticulous clinical records and adherence to the highest professional and legal standards are essential responsibilities.

Key Responsibilities:

  • Conduct thorough mental health assessments and formulate tailored care plans.
  • Deliver evidence-based interventions to service users with diverse and complex mental health needs.
  • Collaborate effectively within a multidisciplinary team environment.
  • Provide comprehensive home visits and community-based support, emphasizing person-centered care.
  • Maintain precise and accurate clinical records in accordance with regulatory requirements.

Requirements:

  • Registered Mental Health Nurse (RMN) - Band 6 qualification is mandatory
  • Proven experience in community mental health settings
  • Current and valid registration with the Nursing and Midwifery Council (NMC) is required
  • Possession of a full UK driving license and access to a reliable vehicle is essential
  • Demonstrated expertise in risk assessment and crisis intervention techniques
  • Exceptional communication and collaborative teamwork skills are required

Working Hours & Pay Rate:

  • Standard working hours: Monday to Friday, 9:00 AM - 5:00 PM.
  • Competitive hourly rate: £26 per hour.
  • Full-time and part-time employment options are available.
