Kingdom Healthcare is currently seeking an experienced and compassionate Registered General Nurse (RGN) who has experience of working as a Band 5 Community Nurse to join the team of Community Nurses at an NHS hospital in Wilmslow.
As a Registered General Nurse (RGN) who has experience of working as a Band 5 Community Nurse, you will provide high-quality nursing care to patients in various settings, such as wards, clinics, and the community.
You will collaborate with other healthcare professionals, including doctors, healthcare assistants, and therapists, to deliver patient-centred care that meets their physical, emotional, and psychological needs.
Responsibilities:

- Patient Assessment and Care Planning:
- Initial Assessments: Conducting thorough assessments of patients' health needs, including physical, emotional, and social aspects
- Care Plans: Developing and implementing personalized care plans in collaboration with patients and other healthcare professionals
- Clinical Nursing Care:
- Medication Administration: Administering prescribed medications and ensuring accurate documentation
- Wound Care: Performing wound dressings, managing chronic conditions (e.g., diabetes), and providing post-operative care
- Monitoring Vital Signs: Checking vital signs such as blood pressure, heart rate, and respiratory rate, and identifying any changes in health status
- Infection Control: Following infection control protocols to ensure safety for patients and staff
- Patient Education and Support:
- Health Promotion: Educating patients and their families about their health conditions, medication management, and healthy lifestyle choices
- Self-Management: Supporting patients in managing long-term conditions and providing guidance on self-care and symptom monitoring
- Collaboration with Other Healthcare Providers:
- Multi-Disciplinary Team (MDT) Working: Collaborating with other healthcare professionals, such as GPs, physiotherapists, social workers, and occupational therapists, to deliver holistic care
- Referrals: Referring patients to specialist services when required
- Care Coordination:
- Care Delivery: Ensuring continuity of care and coordinating follow-up visits to patients, managing patient caseloads efficiently
- Case Management: Overseeing the care of patients, ensuring that they receive the appropriate interventions at the right time
- Record Keeping and Documentation:
- Accurate Record Keeping: Maintaining detailed and accurate patient records, ensuring all interventions, assessments, and plans are properly documented.
- Electronic Records: Using electronic health records (EHR) systems to update patient data and ensure compliance with legal and regulatory standards
- Advocacy and Patient Support:
- Patient Advocacy: Supporting patients in understanding their rights, providing advocacy in decision-making, and ensuring their voices are heard in the care process
- End-of-Life Care: Providing compassionate palliative care to patients nearing the end of life, ensuring comfort and dignity
- Safeguarding:
- Safeguarding Vulnerable Adults and Children: Identifying and responding to potential safeguarding concerns, ensuring the protection of vulnerable individuals
- Supervision and Professional Development:
- Mentorship: Supporting the learning and development of less experienced staff, including healthcare assistants and student nurses
- Continual Professional Development (CPD): Participating in training and development opportunities to enhance nursing skills and knowledge
- Clinical Leadership:
- Leadership within the Team: Providing leadership within the nursing team, promoting a high standard of care, and being a role model in clinical practice
Minimum Requirements:
To be considered for the role of Registered General Nurse (RGN) who has experience of working as a Band 5 Community Nurse at our NHS hospital in Wilmslow, UK, you must meet the following minimum requirements:
- Valid in-date Nursing and Midwifery Council's membership: You must be registered with the Nursing and Midwifery Council (NMC) and have an in-date membership
- At least 6 months' experience working as a Registered General Nurse (RGN) including working as a Band 5 Community Nurse in the UK
- Valid UK visa that permits you to work as a Registered General Nurse (RGN) in the UK
- Vaccinations: You must be vaccinated against Hepatitis B, Measles, Mumps, Rubella, Varicella, Tuberculosis, and COVID-19
- Desirable but not mandatory - Car driver: A valid car driving licence is mandatory - Please note mileage is not paid
- Valid Disclosure and Barring Service (DBS) certificate
- DBS certificate registered on the DBS update service
