Meadow House Hospice is excited to announce the expansion of our palliative care service and we are looking for two dedicated Band 7 Community Specialist Palliative Care Triage and Duty Nurses to join our team. This is a new service for the hospice, so it offers the opportunity for staff to be at the frontline of developing and leading on the way we deliver care to our patients, their families and how we offer support to our partners in caring.We are looking for someone who is passionate about delivering exceptional palliative care. They must be skilled in collaborative working with a variety of healthcare professionals, able to manage complex patient care needs with compassion and efficiency and committed to improving patient outcomes and supporting our service expansion and development. Main duties of the job The successful candidates will collaborate with key stakeholders to ensure that patients referred to our service receive timely and high-quality specialist palliative care. They will work both at the hospice and in the community, visiting patients to undertake assessments in their place of residence as needed. You will be responsible for responding to urgent calls to the hospice and providing immediate advice to our stakeholders and patients and their carers. You will also play a crucial role in ensuring excellent triage into the hospice, helping us deliver on our strategic goals for the residents of Ealing and Hounslow. Job description Job responsibilities To provide an evidence-based Specialist Palliative Care Service to adult residents of Ealing and Hounslow CCGs that is equitable according to need. To work collaboratively with the Primary Care Team, acute sector, social services and other agencies, to establish a close working relationship that is essential in ensuring that care is coordinated and responsive to individual need. The post-holder will be expected to exercise a high degree of personal professional autonomy and make critical judgments to satisfy the expectations and demands of the role. To act as a resource and educator to the primary care team and generic palliative care providers, helping to develop and empower their practice. To demonstrate empathy with patients and carers, ensuring that effective communication is achieved particularly in circumstances where there may be high levels of distress. To manage the triage of referrals into the hospice, determining urgency of response Lead on the role of duty nurse, responding to urgent calls to the hospice from patients known to the community team and from community teams requesting advice on patient management, in the absence of the key worker.
