Hospital Discharge - Long Term Contract - up to £38p/hr

NonStop are currently partnered with a reputable hospital team seeking experienced social workers to be a part of their discharge service.

This role is paying better than any other hospital teams around and can offer you some long term job stablility as a locum social worker.

Your Role:

• Coordinate discharge planning for patients.
• Work with multidisciplinary teams on individualised plans.
• Communicate with patients and families about post-discharge care.
• Ensure compliance with hospital policies.

Requirements:

• Degree in social care
• Social Work England Registration
• Experience in Hospital Discharge team
