About Us: Embark on a rewarding journey with the London Borough of Barnet as a Lead Practitioner within our esteemed Social Care Direct Team. This role presents a distinctive opportunity for a seasoned professional to take the helm in a non-caseholding capacity, steering our team towards efficient handling and direction of referrals received via email. With our hybrid working model, enjoy the flexibility of two days a week working remotely.

Key Responsibilities:

  • Assess and prioritize emails received by Social Care Direct, furnishing insightful notes and guidance to team members on case direction and subsequent actions.
  • Methodically allocate emails to team members, ensuring swift and equitable distribution of workload.
  • Provide invaluable support and counsel to team members regarding assigned emails, aiding in informed decision-making and effective case management.
  • Identify pertinent processes and pathways such as reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge, and offer appropriate signposting.
  • Diligently screen incoming emails to ascertain suitability for referral to Social Care Direct, maintaining meticulous attention to detail and adherence to established protocols.

Requirements:

  • Hold professional qualifications in Social Work and be registered with Social Work England.
  • Demonstrate proficiency in social care practice, particularly in triaging and directing referrals.
  • Possess strong organizational acumen, adept at managing email correspondence and effectively allocating workloads.
  • Exhibit exceptional communication skills, capable of delivering clear guidance and support to team members.
  • Display proficiency in relevant processes and pathways within adult social care, including reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge.
