Our client is seeking a Community Nurse Case Manager to support clients in the Midlands region.� The ideal location for the Nurse Case Manager to live would be around Nottingham, to cover Lincolnshire, Staffordshire, and Midlands.
This role offers a unique blend of home-based work and community visits, ensuring clients can live independently at home and within their wider communities.
This role offers a competitive salary of �48,000 plus the opportunity to earn a bonus. You'll enjoy the flexibility of working from home two days a week, with the remaining three days spent visiting clients in the community. As a Community Nurse Case Manager, your responsibilities will include:
- Conducting initial assessments in the community
- Providing clinical oversight to your team
- Collaborating with the wider multidisciplinary team (MDT)

- Managing a team of non-clinical case managers
