Nurse Case Manager

Our client is seeking a Community Nurse Case Manager to support clients in the Midlands region.� The ideal location for the Nurse Case Manager to live would be around Nottingham, to cover Lincolnshire, Staffordshire, and Midlands.
This role offers a unique blend of home-based work and community visits, ensuring clients can live independently at home and within their wider communities.
This role offers a competitive salary of �48,000 plus the opportunity to earn a bonus. You'll enjoy the flexibility of working from home two days a week, with the remaining three days spent visiting clients in the community. Additionally, you'll have the chance to make a real difference in the lives of those with complex care needs.

As a Community Nurse Case Manager, your responsibilities will include:

• Conducting initial assessments in the community
• Providing clinical oversight to your team
• Collaborating with the wider multidisciplinary team (MDT)
• Attending meetings and making necessary referrals
• Managing a team of non-clinical case managers

The ideal candidate for the Community Nurse Case Manager role will have:

• RGN qualification with excellent clinical skills in complex care
• Experience with peg feeds, ventilators, and tracheostomy management
• A valid driving licence, as the role involves remote and community-based work
• Will live in or around the Nottingham area
