1) One Adult Social Care Occupational Therapist to work in the Integrated Discharge Team at Scunthorpe Hospital.

2) One Adult Social Care Occupational Therapist to work with the NLC Independent Living Team, assessing for equipment, adaptations and supporting care home discharges
