Support Coordinator
We're looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Stroke Recovery Service team providing Key Worker support across Somerset.
This is an exciting, remote working opportunity, to work with stroke survivors and their families to support them following a stroke.
Position:
S11152 Stroke Support Coordinator (x2 posts)
Location:
Home-based Somerset however, frequent travel will be required as part of this role (to include team meetings or other work-related meetings)
Hours:
Full-time, 35 hours per week
Contract:
This is a fixed-term contract until 31 March 2025. Services are contracted and there is currently funding for this contract until 31 March 2025

Interviews will be held via video conferencing. The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.
The Stroke Support Coordinator will:
Support new stroke survivors and their carers from hospital discharge into the community.
Provide personalised information, advice and support.
Support stroke survivors to make informed lifestyle changes, which will help them to prevent further strokes.
Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke.
A caring profession ideally with experience of supporting people with disabilities.
Setting up and running support groups.
This role requires extensive travel across a large geographical locality to visit people at home and hot desk with colleagues in community settings.
