Community Physiotherapist Role
Job Title: Band 6 Physiotherapist
Location: Taunton
Start Date: ASAP
End Date: Ongoing
* The Physiotherapist requires community experience within adult reablement.
* The Physiotherapist must be a car driver and owner. Although the service works in geographical clusters, the individual should be willing to travel cross county if required to meet patient need.
* The D2A service provides short term reablement following hospital admission within peoples own homes.
* The Physiotherapist will work alongside D2A trained provider agencies to deliver assessment and reablement.
* The Physiotherapist will set patient led SMART goals to progress independence and recommend longer term care and support needs.
* Qualified Physiotherapist
