As a Social Worker in the Adult Community Team, you will play a key role in supporting adults to live independently and improve their quality of life within the community. Your work will involve assessing needs, identifying support options, and implementing person-centered plans that promote independence and well-being. You will ensure that individuals receive the appropriate care and resources while safeguarding their rights and dignity.


Responsibilities include:


  • Conduct thorough assessments of adults' needs under the Care Act 2014 and other relevant legislation.
  • Develop and implement personalized care plans aimed at promoting independence and community living.
  • Safeguard vulnerable adults, ensuring their safety and well-being through appropriate interventions.
  • Work collaboratively with multidisciplinary teams, families, and community services to provide holistic support.
  • Advocate for adults' rights and ensure they have access to the necessary resources, services, and benefits.
  • Regularly review and monitor care plans to adapt to changing needs and circumstances.
  • Ensure compliance with statutory responsibilities and best practices in safeguarding and care provision.


Requirements:


  • 2 years experience post qualification
  • Action safeguarding procedures
