Qualified Social Worker for Safeguarding

As a Social Worker in the Adult Assessment and Safeguarding Team, you will play a critical role in safeguarding vulnerable adults and ensuring their safety and well-being. Your responsibilities will include conducting comprehensive assessments to identify risks, implementing appropriate safeguarding measures, and protecting adults at risk of abuse or neglect. You will collaborate closely with individuals, their families, and multidisciplinary teams to ensure that support is effective, person-centered, and aligned with safeguarding protocols.

Responsibilities include:

• Conduct comprehensive assessments of adults' needs in accordance with the Care Act 2014 and other applicable legislation.
• Create and implement personalised care plans designed to promote independence and enhance community living.
• Safeguard vulnerable adults by ensuring their safety and well-being through appropriate interventions.
• Collaborate with multidisciplinary teams, families, and community services to deliver holistic support.
• Advocate for the rights of adults, ensuring they have access to essential resources, services, and benefits.
• Regularly review and monitor care plans to accommodate changing needs and circumstances.
• Ensure adherence to statutory responsibilities and best practices in safeguarding and care delivery.

Requirements:

• 2 years experience post qualification
• Action safeguarding procedures
