Qualified Social Worker

Do you have social work experience of working within Adult services?

We are working with Southwark to source experienced social workers to join the Hospital Discharge Team. The successful Social Worker can expect up to £32 p/h with a full time contract available.

Responsibilities include but are not limited to:

• Cover duty
• Working on INT with a mixed caseload completing assessments
• Support planning and reviews
• Mental capacity assessments and best interest decisions
• Attending active case management MDT's and CHC meetings
• Supporting people with complex needs.
