RESPONSIBILITIES:
- Qualified practitioner with ability to manage caseload within community setting with support and supervision
- Assessment of new referrals to service
- Liaising with clients, carers and families via telephone, video conferencing or face-to-face
- Covering Duty
- Taking part in regular MDTs
SKILLS/EXP REQUIRED:
Experience of working within a community older adult or Memory Service team, specifically in a dementia assessment team/memory clinic either currently working in one or within the past 2 years
Qualified mental health practitioner who has knowledge of working with and managing a caseload autonomously in the community
Not Older Adults services or placements while a student nurse in a OACMHT
