Social worker - Adult's health and commissioning - North

Opus People Solutions on behalf of Cambridgeshire County Council have an exciting opportunity for a Social Worker in Adults health and commissioning to join there Team.
This role will be hybrid based at Hinchingbrooke Hospital with 3 days a week, with possibly more pending demands of the service
In this role you will:

• Discharge to assess may be introduced into the service, therefore external travel throughout the county may be required.
• Referrals are allocated and assessments are to be with brokerage within 4 hours to source care.
• Safeguarding enquiries and information gathering required so experience in this area is mandatory.
• Complete care and support planning with patients
• Complete Care Act Assessments and Mental Capacity assessment.
• Assess and manage risk due to the nature of time specific discharge planning
• Ensure safe and comprehensive discharges taking into consideration our system partners and the pressure on other services, for example preventing delays and ensuring adult social care are not the cause for the delay.
• Operating within a multi-disciplinary setting and seeking evidence from other professionals within the hospitals to inform defensible decision making.
