Job Description:

Are you an experienced social worker with a passion for supporting adults with learning disabilities?
In this role, you will work closely with individuals and their families to assess needs, develop care plans, and ensure they receive the necessary support to enhance their quality of life.

Responsibilities:

• Conducting comprehensive assessments to identify the needs and strengths of adults with learning disabilities.
• Developing and implementing personalized care plans that promote independence, well-being, and social inclusion.
• Working collaboratively with multi-disciplinary teams, external agencies, and community organizations to coordinate care and services.
• Ensuring all interventions comply with relevant legislation, policies, and procedures related to adult social care.
• Building strong, trusting relationships with individuals, their families, and carers to support their long-term needs.

Qualifications:

• Qualified Social Worker registered with Social Work England.
• Proven experience in a social work role, particularly with adults with learning disabilities.
• Excellent communication and interpersonal skills, with the ability to build effective relationships with individuals, families, and professionals.
• Knowledge of relevant legislation, policies, and procedures related to adult social care and learning disabilities.
