About the Role:



As an Adults Community Social Worker, you will work collaboratively with adults who may have physical or learning disabilities, mental health issues, or other support needs. Your focus will be on enabling individuals to lead fulfilling lives, access the right services, and maintain their independence.



Key Responsibilities:




  • Conduct comprehensive assessments of adults' needs and eligibility for services.

  • Develop and implement tailored support plans that reflect individual aspirations and promote independence.

  • Work closely with service users, families, and other professionals to coordinate care and support.

  • Engage in safeguarding practices, ensuring the safety and well-being of vulnerable adults.

  • Maintain accurate case records and documentation in compliance with statutory requirements.

  • Advocate for the rights and needs of service users, ensuring they are at the heart of decision-making.

  • Participate in community outreach and engagement initiatives to strengthen support networks.



Requirements:




  • Qualified Social Worker (Degree in Social Work or equivalent).

  • Registration with Social Work England.

  • Experience in adult social work, particularly in community settings.

  • Strong knowledge of relevant legislation, including the Care Act 2014 and safeguarding procedures.

  • Excellent communication, interpersonal, and assessment skills.

  • Ability to work both independently and as part of a multi-disciplinary team.

  • A commitment to promoting equality, diversity, and inclusion in practice.
