NonStop Consulting are partnered with an award-winning hospital team in South London, seeking experienced Adult Social Workers with Hospital experience to join their team.

Responsibilities:

  • Participate in Discharge to Assess duty service
  • Manage a caseload, including safeguarding enquiries
  • Conduct Care Act and Mental Capacity Assessments
  • Implement care packages per guidelines
  • Collaborate with health and community colleagues
  • Engage in Continuing Professional Development (CPD)
  • Maintain Social Work England standards

Qualifications & Skills:

  • SWE registered Social Worker
  • Degree in social care
  • Experience in hospital discharge, Care Act Assessments, Mental Capacity Assessments, and safeguarding
  • Strong communication and time management skills
  • Knowledge of relevant legislation and frameworks
  • Team-oriented, collaborative, and committed to high service standards
