Social Worker - Support hospital discharges from acute settings

Requirements for the role below:

• Qualified Social Worker with experience in hospital discharge planning.
• Strong knowledge of the Care Act, Mental Capacity Act, and Safeguarding procedures.
• Ability to work in a fast-paced assessment environment with excellent time management skills.
• A pragmatic and flexible approach to travel and hybrid working.

We are seeking an experienced Social Worker to join our Hospital Discharge & Assessment Team. This is a fast-paced role, requiring expertise in hospital discharge planning, strengths-based assessments, and care planning. As a Social Worker you will play a crucial role in supporting patients as they transition from hospital to appropriate care settings.

Key Responsibilities:

• Conduct Strengths-Based Assessments and develop person-centred care plans.
• Support and lead Best Interest Meetings and Mental Capacity Assessments.
• Facilitate safe and timely hospital discharges while ensuring compliance with relevant legislation.
• Work closely with multidisciplinary teams, including health professionals and external agencies.
• Support safeguarding processes where required.
