Roles available

Pathways

The team provides an assessment of needs for young people who are preparing to leave care and the development of a pathway plan to meet these needs. The team works closely with partners as corporate parents to support children to develop their life skills to achieve their full potential and work towards their aspirations.

Children in our Care Teams

Following care proceedings, there is a plan for every child or young person to remain living outside of the family; they will be allocated a social worker from one of these teams. The Children in Our Care teams also have the support of Resources Workers who help social workers to complete Life Story work for our children and young people to understand their journey's and also support facilitating and supervising family time.

Children In Our Care Social Workers will work with our children and young people towards making sure their care plans are progressed.

Safeguarding and Care planning teams

The Safeguarding and Care Planning Teams work with families under the auspices of Child in Need and Child Protection. The teams work collaboratively with families to support them to make changes to ensure that their children are safe and that their needs are met.

Children and young people who become cared for by the local authority will have a looked after care plan. For some children and young people where this plan means they will remain cared for, this will mean they are allocated a new social worker from the Children Looked After Team, however for some children and young people their long-term care plan may be one of adoption. We continue to support and work with these children until they move and are settled with their adoptive families.

Within the SCP teams, there are also specialist teams working with children and young people at risk of harm outside the home, such as criminal and sexual exploitation (Aspire Team). A team working with parents, where there is a risk or likelihood of unmet needs for their unborn (Pre-birth Team).

Assessment teams

Assessment Teams work with children, young people and their families where referrals into children's social care have identified a need for Level 4 support. Social workers in the Assessment Teams complete S47 visits to children following strategies. Progress children's planning to Initial Child Protection Conferences and Public Law Outline where required prior to transfer to the SCP teams. There is some initial court work undertaken, however this is minimal as involvement transfers to the SCP teams at the first hearing.
Direct work with children and young people is an integral part of the Assessment Teams practice as this helps us to understand children and young peoples' daily experiences. The Assessment Teams work collaboratively with children, young people and their families and also with agency professionals as part of producing robust meaningful assessments of children and young people's needs.
