Job overview An exciting opportunity has arisen for a 12-month fixed term role for an experienced and enthusiastic pharmacist to become part of our multi-morbidity model of care (MMMoC) project to optimise medicines for patients with chronic kidney disease and cardiometabolic disorders across South East London. The project is a collaboration between primary and secondary care colleagues to create a person-centred, holistic integrated model of care for people with multiple long-term conditions to improve patient experience, change ways of working and increase efficiencies. This post is one of two roles hosted by King s College Hospital NHS Foundation Trust and Guy s and St Thomas NHS Foundation Trust and will work across multiple sites including Lewisham and Greenwich NHS Trust and primary care. This post will have responsibility for medicines optimisation in the MMMoC project and support integration of services and pathways across the interface for renal, diabetes and cardiovascular specialties. Main duties of the job The successful candidate will: have significant experience as a specialist clinical practitioner in chronic kidney disease and/or cardiovascular disease and/or diabetes be able to demonstrate a commitment to patient care a ct as a role model in the CKD or cardiometabolic field be able to demonstrate the ability to influence at consultant and senior leadership level be able to demonstrate experience of service development and improvement across a system wide multi-disciplinary environment have excellent leadership, facilitation and communication skills be able to demonstrate the ability to work innovatively work with the Principal Pharmacist and Lead Pharmacist, to extend the boundaries of professional practice within chronic kidney disease, cardiovascular and diabetes. Candidates must have successfully completed the Postgraduate Diploma in Clinical Pharmacy Practice (or equivalent) and be a registered Independent Prescriber. Act as a specialist role model and an expert in the medical management within chronic kidney disease, cardiovascular and diabetes. Support the leadership of clinical pharmacy services in chronic kidney disease, cardiovascular and diabetes in conjunction with the Lead Pharmacist. Participate regularly in outpatient clinics to deliver and promote high quality, cost-effective and patient focused pharmaceutical care. Act as a pharmacist independent prescriber to optimise the care of patients within chronic kidney disease, cardiovascular and diabetes. Ensure a high level of clinical pharmacy input to the care of patients within chronic kidney disease, cardiovascular and diabetes. Providing interface and discharge liaison and support. Actively participate in multidisciplinary ward rounds, delivering and promoting high quality, cost effective, patient-focused pharmaceutical care. Provide patient and carer education with close liaison across the interface. Promote safe drug use within chronic kidney disease, cardiovascular and diabetes specialties. Development, implementation and audit of local guidelines. Supervise and train pharmacy staff. Provide education and training of healthcare professionals within and outside the Trust. Regular financial reporting, drug usage and expenditure forecasting, and drug budget management. To participate in practice and clinical research and audit. To work with the Principal Pharmacist and Lead Pharmacist, to extend the boundaries of professional practice within chronic kidney disease, cardiovascular and diabetes.
