Salary: £25,702 per annum

Hours: 37.5 hours

Job Type: Permanent

Shift: Monday to Friday - 9am to 5pm

About the role

We are looking to recruit several reablement support workers to provide specialist expertise and support to Living Well Centre teams. They will facilitate the achievement of person-centred outcomes for people living in the community, some of whom have complex mental health and other support needs. Support workers will work collaboratively with professionals, including care coordinators, social workers, occupational therapists, psychologists and psychiatrists as detailed in their support plans to achieve agreed outcomes and provide the best practice support for clients.

The support workers will act as ‘social prescribers’ connecting people to local groups and services including peer support. They will also help people reconnect with family and friends. The post holder will be expected to develop and retain a wide-ranging knowledge of local provision to ensure that they are aware of the full range of options available which will be responsive to individuals’ wider social, cultural and emotional needs.

The post holder will be responsible for working directly with people in the community, adopting a person-centred and strengths-based approach. They will provide support to people in line with the clinical pathways and best practice within the LWNA, working jointly with other staff members and wider professionals such as GPs where required.

Duties will include

- They will provide intensive reablement support through practical hands-on work with and collaborate with people to complete EDL.
- Household chores such as shopping, cooking and cleaning.
- Support to help people re-learn domestic tasks from a strengths-based perspective.
- Support workers will assist with a range of reablement needs which include domestic support, substance misuse and harm minimisation, physical health concerns, advice around Social Care and care packages, benefits and money management.

About you

The Lambeth Living Well Network Alliance (LWNA) commenced operations on 1st July 2018 and was developed to transform the way that adult mental health services are delivered in Lambeth with the vision that ‘every citizen, whatever their abilities or disabilities, can flourish, contribute to society and lead the life they want to lead’

This is a partnership between Certitude, Lambeth Council, Thames Reach, Lambeth Clinical Commissioning Group and the South London and the Maudsley NHS Trust (SLaM). Together partners support the achievement of the LWNA ‘three big outcomes’ identified by local people: to support people to recover and stay well, make their own choices and participate on an equal footing in the community

The LWNA has developed services to meet borough mental health needs including the Single Point of Access (SPA) which opened in Autumn 2019-  an assessment service which allows people to refer themselves or be introduced for help by GPs or other professionals. Individuals have a pathway in which they can speak to a clinician and be referred directly to the most appropriate support, in a timely manner that avoids repetition.  At the same time, the SPA commenced operations, the alliance brought together community mental health teams, social workers and voluntary sector staff together into two teams. These teams are the Short Term Support Service (who usually support people for up to three months) and Focused Support Service (who support people longer term.) The team are based in three Living Well Centres (LWCs): North, South East (both currently Brixton) and South West (Streatham)
